Aetna updated its coverage policy for Ixabepilone (Ixempra) effective April 7, 2026, establishing specific medical necessity criteria for breast cancer treatment. The policy covers ixabepilone as monotherapy for HER2-negative disease, as fourth-line combination therapy with trastuzumab for HER2-positive disease, and in combination with capecitabine for anthracycline/taxane-resistant disease with specific liver function requirements.
