Summary

Action Required
Immediately: Ensure all nursing facility, ICF-ID, and hospice LOC entries are completed in KLOCS prior to resident admission and PASRR evaluations. Update facility procedures to require timely discharge/transfer of members in KLOCS to avoid reimbursement delays. Document all backdate/correction requests with written justification including system error messages or help desk ticket numbers. Contact KLOCsOrgAdmin@ky.gov if KLOCS administrator changes are needed. Failure to follow guidelines may result in DMS corrective action and payment delays.
