Summary
Starting January 1, 2025, EyeMed will manage prior authorization for two injection codes (J2777 and J0177) that were previously managed by Healthfirst. These codes are for eye injections used to treat retinal conditions.
Action Required
Before January 1, 2025: Billing team must update prior authorization workflows to submit requests for codes J2777 (faricimab-svoa injection) and J0177 (aflibercept hd injection) through EyeMed instead of Healthfirst. Update billing system to reflect new prior auth vendor. Train staff on EyeMed's prior authorization process and requirements.
