Summary
Medical Mutual has discontinued acceptance of paper Provider Action Request (PAR) forms by mail effective December 1, 2025. All PAR forms must now be submitted through the Availity provider portal under the Medical Mutual payer space Resources tab.
Action Required
Before December 1, 2025: All staff must stop mailing Provider Action Request forms and transition to electronic submission through Availity portal. Register for free Availity account at https://www.medmutual.com/Provider if not already done. Access PAR forms in Medical Mutual payer space under Resources tab. Only bulk medical records/CDs should continue to be mailed to Member Appeals and Grievances Department. All other paper documents will be returned and must use Availity portal.
