Excellus BlueCross BlueShield has updated their lumbar microdiscectomy policy effective October 15, 2025, establishing comprehensive criteria for initial and repeat procedures including specific conservative treatment requirements, imaging concordance, and mental health screening. The policy also explicitly excludes laser techniques and certain sole indications as not medically necessary.

Action Required

Action needed
By October 15, 2025: Prior authorization team must update lumbar microdiscectomy criteria to require ALL specified conservative treatments (6 weeks each of medications, physical therapy, and/or epidural injections), concordant MRI/CT findings, and absence of unmanaged mental health disorders. Providers must document functional limitations, specific symptoms, physical exam findings, and failed conservative care for both neurogenic claudication and radiculopathy cases. For repeat procedures, ensure 12+ weeks have elapsed since prior surgery. Claims will be denied if comprehensive documentation requirements are not met.
