Providers must document functional limitations, specific symptoms, physical exam findings, and failed conservative care for both neurogenic claudication and radiculopathy cases. For repeat procedures, ensure 12+ weeks have elapsed since prior surgery. Claims will be denied if comprehensive documentation requirements are not met.
