Summary
Excellus BlueCross BlueShield updated their IOL implant policy effective October 16, 2025. Monofocal IOLs are considered medically appropriate after cataract extraction, while multifocal, toric (astigmatism-correcting), and accommodating/trifocal IOLs are considered not medically necessary. Patients can still choose premium IOLs but must sign a waiver and pay the additional cost beyond standard monofocal IOL reimbursement.
