Summary
CMS now requires all Medicare Special Needs Plan (SNP) providers to complete annual Model of Care (MOC) training. UPMC Health Plan has created an online training module that providers must complete using their tax ID number.
Action Required
Immediately: All providers serving Medicare SNP patients must complete the annual MOC training at upmchp.us/snpmoc using their tax identification number. Contact Provider Services at 1-866-918-1595 with questions. This is a CMS requirement that must be completed annually to maintain SNP provider status.
