Summary
Health Net will no longer accept or reimburse Medicare add-on codes G2211 and G2212 effective April 1, 2026. Providers must use standard E/M codes (99202-99205, 99211-99215) instead to reflect the level and duration of services provided.
Action Required
Before April 1, 2026: Billing team must update billing software to discontinue use of CPT codes G2211 and G2212. Update encounter forms and EMR templates to use standard E/M codes 99202-99205 and 99211-99215 instead. Train providers on proper E/M code selection to accurately reflect service level and duration. Claims submitted with G2211 or G2212 on or after April 1, 2026 will be denied.
