Summary
Starting September 1, 2026, UnitedHealthcare will exclude certain brand-name injectable oncology drugs from medical benefit coverage when generic alternatives are available. Prior authorization will still be required for both excluded drugs and their generic alternatives.
Action Required
By September 1, 2026: Billing team must update system to flag excluded injectable oncology drugs (J0185, J1434, J2468, J9054, J9072, J9074, J9076, J9172, J9292, J9304, J9324, J9999) and transition to generic alternatives. Update prior authorization workflows to include both excluded and alternative drugs. Providers must transition existing patients from excluded drugs to covered alternatives when current authorizations expire.
