Summary
Starting September 1, 2026, Medi-Cal hospice providers must submit their Hospice Program Attestation Forms through the Medi-Cal Provider Portal instead of the current web-based system, which will be permanently shut down. Providers without portal access will be unable to submit required attestation forms.
Action Required
Before September 1, 2026: Hospice billing teams must register for Medi-Cal Provider Portal account if not already registered. Visit New Medi-Cal Providers webpage for registration instructions. Providers currently billing DHCS directly should already have portal access. Contact MCHospiceClerk@dhcs.ca.gov for support. Failure to register will prevent submission of required attestation forms.
