Summary
Capital Blue Cross clarified that CPT 77332 (treatment device) should NOT be billed separately for HDR gynecologic brachytherapy procedures because it is already bundled into insertion codes CPT 57155 and 57156. While CPT 77332 remains on the prior authorization list, requests will not be approved when submitted with these HDR procedures.
Action Required
Immediately: Billing team must stop billing CPT 77332 separately with HDR gynecologic brachytherapy insertion procedures (CPT 57155 or 57156). Update billing software to prevent bundling these codes together. Continue billing only CPT 57155/57156 for HDR procedures using vaginal cylinder or tandem and ovoid applicators. Separate billing of CPT 77332 with these procedures will result in denial.
