CareFirst has transitioned PCP panel rosters and care plans for CHPMD and Advantage DualPrime members from MyHealth Portal to a new Population Health Manager tool in the CareFirst Provider Portal. The MyHealth Portal will be retired after January 30, 2026, with no updates after January 1, 2026.
Action Required
By January 1, 2026: All staff must transition from MyHealth Portal to the new Population Health Manager tool in CareFirst Provider Portal for accessing PCP panel rosters and care plans. Complete training using the Population Health Manager tutorial in the Learning and Engagement Center. Stop relying on MyHealth Portal data after January 1, 2026 as it will no longer be updated, with full portal retirement on January 30, 2026.
