Summary
Point32Health (Harvard Pilgrim/Tufts) is implementing multiple Medical Necessity Guideline updates with new prior authorization requirements for various procedures including foot reconstruction, gender affirming services, therapeutic apheresis, and Medicare-covered services. Additionally, some existing procedures like esophageal dilation will now require appropriate ICD-10 codes for coverage without prior auth.
Action Required
Before June 1, 2026: Billing team must update prior authorization workflows for multiple new requirements including foot reconstruction procedures (CPT 28308, 28310, 28298, 28750), gender affirming services when billed with gender dysphoria diagnosis (CPT 21175, 21270, 53430, 53450), therapeutic apheresis (CPT 36514, 36516), and various Medicare services. Update billing system to ensure CPT 43249 is submitted with appropriate ICD-10 codes. Front desk staff must verify prior auth requirements before scheduling affected procedures.
