Summary

Action Required
By December 15, 2019: Providers in VIP Prime Network must complete EmblemHealth SNP Model of Care training online using credentials sent via email/fax/mail, or download training materials from provider portal. Failure to complete will result in removal from EmblemHealth Provider Directory. Immediately: Providers with multiple EFT bank accounts must re-enroll with PNC for electronic payments. For Bridge Network members, enroll separately with ECHO Health for payments (contact 888-492-0032). Primary care providers should regularly check panel reports and schedule new patient visits during open enrollment, encouraging Medicare/SNP members to complete Health Risk Assessments.
