Action Required
Before December 31, 2010: Billing team must transition from CPT code 99070 to HCPCS code A4264 for permanent implantable contraceptive devices. Update billing software to use A4264 for these procedures. Include HFS 2189 Sterilization Consent Form with all insertion procedure claims. Add V25 series diagnosis codes for Illinois Healthy Women program participants. Claims using 99070 after December 31, 2010 will be rejected.
