Summary
Nevada Medicaid is sunsetting Provider Type 82 (Behavioral Health Rehabilitative Treatment) and requiring providers to transition to new provider types. Providers have 30 days after approval under their new provider type to submit new prior authorization requests using form FA-11, reference original PA numbers, and transfer remaining units to new authorizations.

Within 30 days of new provider type approval: Behavioral health providers must submit new form FA-11 (Behavioral Health Outpatient or Rehabilitative Authorization Request) for prior authorizations, reference original PA numbers on new requests, request transfer of remaining units from PT 82 authorizations, and update billing system with new authorization numbers for claims.
