Nevada Medicaid has updated all multi-page forms to include recipient name, date of service, and page number on each page per documentation policy requirements. Providers must immediately switch to the new forms, as submissions using old forms will be denied starting August 1, 2025.

Immediately: Download and implement all updated multi-page forms from Nevada Medicaid Provider website Forms page. Billing team and providers must stop using previous form versions for all Nevada Medicaid submissions. Before August 1, 2025: Ensure all staff are trained on new form requirements including recipient name, date of service, and page numbers on each page. Failure to use updated forms will result in technical denials after August 1.
