Summary

Effective July 1, 2026, Carelon Behavioral Health will no longer provide behavioral health services to Providence Health Plan members. Services will transition directly to Providence Health Plan and network partners. Providers contracted with Providence or participating in Cigna's PPO/First Choice networks will remain in-network. Members will receive new ID cards with updated claims submission addresses.

By June 30, 2026: Billing team must prepare for behavioral health claims transition. (1) Update billing system to reflect new claims submission addresses for Providence Health Plan behavioral health services based on member plan type and network. (2) Verify your practice's network status with Providence Health Plan at 800-878-4445 to confirm whether you remain in-network post-July 1. (3) Stop sending behavioral health claims to Carelon Behavioral Health (PO Box 1850, Hicksville, NY 11802-1850) and direct all claims to Providence Health Plan network partners or Providence directly per updated ID card instructions. (4) Train billing staff on new claim submission processes and routing. (5) Update internal documentation and templates to reflect new claim submission addresses. Failure to route claims correctly will result in claim delays or denials. Members out-of-network post-transition will receive transition support letters; flag these accounts for potential out-of-network processing.
