Summary
Humana Medicare Advantage has revised its prostatectomy coverage policy (effective 07/01/2026) to clearly define medical necessity criteria: prostatectomy is covered for localized prostate cancer (T1-T3) without fixation OR salvage therapy following radiation/brachytherapy/cryotherapy failure, provided patients have ≥10 year life expectancy, no distant metastases, and no regional lymph node involvement. Nerve grafts (CPT 64999) to prevent erectile dysfunction remain non-covered. Two new laparoscopic prostatectomy codes (55869, new effective 1/1/2026) have been added to the covered procedure list.
Action Required
By June 1, 2026: Billing and prior authorization teams must update Humana Medicare Advantage authorization protocols in billing systems to enforce the revised coverage criteria for prostatectomy codes 55810, 55812, 55815, 55840, 55842, 55845, 55866, 55868, and 55869. Specifically: (1) Update authorization rules to require documentation of localized prostate cancer stage (T1-T3) without fixation OR salvage therapy indication; (2) Require verification of ≥10 year life expectancy, absence of distant metastases, and absence of regional lymph node involvement before claims are processed; (3) Implement automated denial logic for CPT 64999 (nerve graft) with explanation that this service is not medically necessary for erectile dysfunction prevention under this plan. Update provider order entry systems and encounter templates to prompt documentation of these medical necessity criteria. Inform urology providers of updated requirements. Effective 1/1/2026 (immediately for new code tracking): Add CPT 55869 (laparoscopic prostatectomy with bilateral pelvic lymphadenectomy) to billing system. Claims submitted without required medical necessity documentation will be denied.
