Summary
CMS will send annual Health Outcomes Survey to Medicare members from mid-July through early November. Providers should encourage member participation when asked about the survey and discuss survey topics throughout the year as appropriate.
Action Required
Starting mid-July 2026: When Medicare members ask about the Health Outcomes Survey, encourage them to respond. Throughout the year, discuss relevant health topics with members including physical health, mental health, physical activity, bladder control, fall prevention, and provider relationships as appropriate during visits.
