Summary
CMS has updated two Medicare forms that hospitals must provide to Medicare beneficiaries: the Important Message from Medicare and the Detailed Notice of Discharge. Hospitals must transition to using the new versions by May 15, 2026, and these forms will be effective until March 31, 2029.

By May 15, 2026: Hospital staff must download and implement the updated Important Message from Medicare and Detailed Notice of Discharge forms from CMS website (available in English and Spanish). Update hospital discharge processes to use new forms. The Important Message must be provided to all Medicare inpatients, and the Detailed Notice only when a beneficiary requests an appeal.
