Amzeeq® (minocycline) Topical Foam (Revised)

By July 23, 2025: Billing and clinical teams must implement prior authorization requirements for all Amzeeq (minocycline) topical foam claims submitted to Humana Medicare Advantage. Before processing or submitting claims, verify that patient records document: (1) treatment for non-nodular moderate to severe acne vulgaris, (2) prior treatment failure, intolerance, or contraindication with a topical retinoid (e.g., tretinoin non-micro or adapalene 0.1% gel), and (3) prior treatment failure, intolerance, or contraindication with a non-delayed release oral antibiotic (minocycline or doxycycline monohydrate 50mg or 100mg). Update prior authorization submission templates and provider education materials to reflect these new step-therapy requirements. Communicate with dermatology providers and prescribers that Amzeeq requires documented sequential therapy attempts before approval. Claims submitted without complete documentation of prior therapy attempts will be denied. Approvals are valid for the plan year duration with renewal available for plan year duration.
