Summary
Effective June 18, 2026 (retroactive to Jan 1, 2026), three new hydrophilic catheter codes (A4295, A4296, A4297) will be added to Indiana Medicaid's DME specialty 250 and must be obtained through the IHCP-contracted vendor for fee-for-service members. Claims with third-party payments are exempt from the vendor requirement.
Action Required
By June 18, 2026: Billing team must update system to route urological catheter codes A4295, A4296, and A4297 through IHCP-contracted vendor for Indiana Medicaid FFS members only. Update encounter forms and staff training to identify these codes. Claims not processed through contracted vendor will be denied unless there is third-party payment indicated.
