Immediately: Billing team must audit all pending and recent oxaliplatin claims to ensure they match only the approved indications listed in CPB 0683. Update billing system rules to flag claims for non-covered indications (breast cancer, cervical cancer, endometrial cancer, head and neck cancer, hepatocellular cancer, melanoma, mesothelioma, non-small cell lung cancer, prostate cancer, urethral cancer) as experimental/investigational and route for medical necessity review before submission. Verify all oxaliplatin claims use HCPCS code J9263. Educate oncology providers that PIPAC (pressurized intra-peritoneal aerosol chemotherapy) with oxaliplatin and chemo-protective interventions for peripheral neuropathy prevention are not covered. Note: Eloxatin brand has been discontinued by Sanofi Aventis US—verify drug availability with pharmacy. Claims submitted for unapproved indications will be denied.
