By 2026-07-01: Billing team must implement the following controls in billing software and encounter systems: (1) APPROVE claims for S8270 (enuresis alarm) ONLY when documentation confirms member age ≥7 years, bedwetting ≥3 nights/week in prior month OR ≥1 episode/week for 1 year, no daytime wetting, and physician exam ruling out organic causes. (2) APPROVE claims for J2597 (desmopressin) ONLY for children >5 years when non-pharmacologic therapies (alarm, fluid/food advice) have failed or member refuses/cannot adhere to alarm treatment. (3) DENY all claims with CPT codes 42820, 42821, 42825, 42826, 42830, 42831, 42835, 42836 (tonsillectomy/adenoidectomy for enuresis), 90880 (hypnotherapy), 97124 (massage/Tuina therapy), 97810-97814 (acupuncture), 98940-98943 (chiropractic), E0720 and E0730 (TENS devices), and J0735 (clonidine) when billed for enuresis indication. (4) Update ICD-10 requirement to N39.44 for all covered enuresis claims and reject claims missing this diagnosis. (5) Create prior authorization workflow requiring physician documentation of medical necessity criteria before approval. Providers must be notified that claims for experimental/investigational therapies will be denied. Failure to implement these controls will result in claim denials and potential overpayment recovery.
