Summary

Action Required
By October 31, 2026: (1) Billing team must update prior authorization system to require 12-month authorizations for ranibizumab/biosimilar claims for diabetic macular edema, diabetic retinopathy, neovascular wet AMD, macular edema following RVO, and myopic CNV. (2) Configure system to enforce maximum dosing limits: 0.3mg per affected eye monthly for DME/DR; 0.5mg per affected eye monthly for RVO/wet AMD; 0.5mg per affected eye monthly for up to 3 months for myopic CNV. (3) Add requirement to clinical notes template for ophthalmologists to document positive clinical response (BCVA improvement, visual field stability, or reduced vision decline rate) for reauthorization claims. (4) Route all ranibizumab claims through step therapy process per linked BC PA Step Therapy Guide. (5) Update claim scrubbing rules to reject claims billed for non-FDA-approved indications (mark as experimental/investigational). (6) Train billing staff that this policy applies ONLY to BlueCross BlueShield of Tennessee members; verify member plan before applying criteria. Failure to implement will result in claim denials and delays in patient access to covered therapy.
