Summary
Indiana Medicaid has removed five incontinence, ostomy and urological supply codes (A4360, A4436, A4437, A5056, A5057) from contracted vendor requirements for fee-for-service members. These codes can now be billed directly without going through contracted vendors.
Action Required
Immediately: Billing team must update billing system to allow direct billing of codes A4360, A4436, A4437, A5056, and A5057 for Indiana Medicaid fee-for-service members without requiring contracted vendor routing. Update vendor routing rules and staff training materials. This change is retroactive to January 1, 2024, so review and resubmit any previously denied claims for these codes.
