Action Required
Immediately (by 2025-12-18): Billing team must implement the following: (1) Update billing system to DENY all Group 2 POV claims (K0806, K0807, K0808) as not medically necessary per new policy language. (2) Create a pre-authorization checklist requiring providers to document: (a) specific MRADL impairment (toileting, feeding, dressing, grooming, bathing), (b) why cane/walker is insufficient, (c) upper extremity function assessment, (d) home access/maneuverability adequacy, (e) patient willingness to use device, (f) caregiver availability (for manual wheelchairs). (3) For power wheelchairs and scooters: add weight class validation rules (e.g., Heavy Duty POV only for 285-450 lbs, Very Heavy Duty 428-600 lbs, Extra Heavy Duty 570+ lbs). (4) Add claim denial rule: reject any wheelchair/scooter claim lacking documentation of home-use intent; deny claims for outdoor-only use. (5) Update encounter templates in EMR to include mandatory fields for all criteria listed in policy section I.B and I.C. (6) Train billing staff to request missing documentation before claim submission. Failure to implement will result in increased claim denials and potential recoupment of improperly paid claims.
