Summary
Effective July 1, 2026, Blue Cross Blue Shield of Montana will implement enhanced claim editing for evaluation and management (E&M) services on commercial plans. Claims will be automatically reviewed against AMA guidelines, and if the documentation doesn't support the billed E&M level, reimbursement will be reduced to the validated lower level of service.

By July 1, 2026: Providers must ensure E&M documentation meets AMA guidelines for medical decision-making complexity before billing. Billing team should educate providers on proper E&M documentation requirements and establish process to submit medical records when reimbursement is reduced to support original claim level. Claims with insufficient documentation will result in automatic downcoding and reduced reimbursement.
