Immediately: Community providers in Maryland must establish contact with Telligen's Maryland Call Center at MarylandUCSupport@telligen.com or (888) 276-7075 to request PASRR Level I screening tools when nursing facility placement is needed. Staff must be trained to complete PDF forms and submit with medication lists and physician progress notes via secure email. This only affects community admissions - acute hospitals and nursing facilities should continue using electronic Qualitrac system.
