Summary

Action Required
By September 30, 2025: Nursing facility billing teams must stop using fax submissions for MDH 257 doctor's orders. Starting October 1, 2025: Submit all doctor's orders through Telligen's Qualitrac system using the new '257' review type. Ensure doctor's orders specify nursing facility services are required and contain clear, readable signatures. Attend Telligen training sessions on September 25th or 29th (1:00-2:30 PM EST). Failure to transition will result in rejection of faxed submissions.
