All providers must verify their directory listing information by November 1, 2025, to ensure patients can accurately find them through the updated system.
Action Required
By November 1, 2025: All providers must review and verify their listing in the Maryland Medicaid Provider Finder at https://health.maryland.gov/mmcp/Pages/provider-finder.aspx. Check that all information is accurate including address, contact details, and HealthChoice MCO affiliations. For general updates, submit supplemental ePREP application. For Type 1 rendering providers, update NPPES account for address changes. For incorrect MCO affiliations, email both mdh.healthchoiceprovider@maryland.gov and mdh.providerfinder@maryland.gov. Allow 30-60 days for changes to be reflected.
