Maryland nursing facilities must report quality assessments for FY 2026 with new rates: $32.92 per non-Medicare patient day for most facilities, or $3.04 for the top 5 Medicaid facilities. Amendment requests are now limited to the two preceding fiscal years only.

For nursing facilities in Maryland only: Use the Cognito reporting tool to submit quality assessment reports for FY 2026. Calculate payments using $32.92 per non-Medicare patient day (or $3.04 if you're one of the top 5 Medicaid facilities from FY 2025).
