MassHealth has updated the SACA-2 application form for seniors and people needing long-term care services. The March 2025 version removes third-party recovery questions, updates immigration documents and department language, and adds new information sharing and estate recovery language.

Direct applicable patients (age 65+ or needing long-term care) to use the updated form available at mass.gov/how-to/apply-for-masshealth-coverage-for-seniors-and-people-of-any-age-who-need-long-term-care or encourage online application at MAhealthconnector.org. Previous versions will still be accepted but using current forms ensures faster processing.
