Immediately: Front desk staff should direct MassHealth patients under 65 (without long-term care needs) to use the updated March 2025 ACA-3 application form available at mass.gov/lists/masshealth-member-applications. Patients 65+ or needing long-term care should use the SACA-2 form instead. Update patient information packets with new application links.
