By May 30, 2025: If your practice is part of an ACPP or PCACO and wants to join/leave an ACO network, work with your ACO to submit required forms to MassHealth. Contact your ACO's MassHealth contract manager for submission requirements. Beginning January 1, 2026: Provide 90-day continuity of care for new ACO members, including payment to out-of-network providers during transition and special attention to pregnant patients, complex medical conditions, and behavioral health services.
