Immediately: Billing team must ensure EAP services are billed using CPT code 99404 only (not behavioral health codes like 90834 or 90847). Providers must obtain EAP codes before providing services by calling 800.926.2273. Submit claims electronically using Payor ID 62308 or mail to Evernorth PO Box 188022, Chattanooga, TN 37422. Providers must call 800.926.2273 to close EAP cases after completion. Failure to use correct CPT code will result in incorrect copayment charges to patients.
