Summary
Texas has extended the 95-day filing deadline and 120-day appeal deadline for oncology radiation treatment codes 77402, 77407, and 77412 for services on or after January 1, 2026. TMHP will automatically reprocess affected claims, so providers do not need to appeal unless claims are denied for other reasons after reprocessing.
Action Required
Monitor reprocessing of claims with CPT codes 77402, 77407, and 77412 for dates of service on or after January 1, 2026. Do not appeal these claims unless they are denied for additional reasons after TMHP completes automatic reprocessing.
