Summary
Effective July 1, 2026, Blue Cross Blue Shield of Oklahoma will implement enhanced claims editing for E&M services on commercial plans, automatically reducing reimbursement to validated service levels when billed services don't support the level claimed. Providers can submit medical records if they disagree with the reimbursement adjustment.
Action Required
Before July 1, 2026: Billing team and providers must review and strengthen E&M documentation practices to ensure all office, inpatient, and outpatient E&M services meet AMA guidelines for the level billed. Update EMR templates to include comprehensive documentation prompts for medical decision-making. Train providers on proper E&M coding to avoid automatic downgrades. Claims will be automatically adjusted to lower service levels if documentation doesn't support the billed level.
