Summary
Indiana Health Coverage Programs (IHCP) has discontinued coverage for Makena (hydroxyprogesterone caproate) injections following FDA withdrawal of approval. HCPCS code J1726 will no longer be reimbursed by Indiana Medicaid effective January 1, 2024.
Action Required
Immediately: Billing team must stop submitting claims with HCPCS code J1726 (Makena injections) for Indiana Medicaid patients with dates of service on or after January 1, 2024. Remove J1726 from encounter forms and billing system templates for IHCP patients. Claims submitted with this code will be denied.
