Indiana Medicaid will separately reimburse FQHCs and RHCs for LARC (long-acting reversible contraceptive) devices, carving them out of the PPS rate. Providers must bill LARC devices on separate claims using specific HCPCS codes and cannot include them with T1015 encounter codes.

Immediately for claims with dates of service on or after Nov 1, 2022: Billing team must separate LARC device billing from encounter visits. Bill LARC devices using codes J7296, J7297, J7298, J7300, J7301, J7306, or J7307 on separate claims. Do not include T1015 codes with LARC devices. Update billing system to prevent combining these codes. Claims with T1015 and LARC codes together will be denied and wrap payments recouped after CMS approval.
