Summary
Indiana Medicaid is removing Accu-Chek Aviva and Accu-Chek Aviva Plus test strips from their preferred diabetes supply list due to manufacturer discontinuation. Current users can continue receiving strips without prior authorization through December 31, 2022, or switch to preferred monitors at any time.
Action Required
By November 13, 2020: Billing team must identify Indiana Medicaid patients using Accu-Chek Aviva, Accu-Chek Aviva Plus, or Accu-Chek Nano SmartView monitors. Notify providers to discuss switching patients to preferred monitors (FreeStyle, Accu-Chek Guide, or TRUE METRIX systems) or document grandfathered status for continued coverage through December 31, 2022. Update preferred vendor lists in billing system to reflect current PDSL.
